What To Expect (Job Responsibilities)

• Build trusted relationships to empower patients in managing their health
• Engage with patients via calls, texts, and emails to facilitate care and address health challenges
• Collaborate with physicians, nurses, and care teams to enhance care coordination and outcomes
• Accurately document interactions in the electronic health record (EHR) for seamless communication
• Travel occasionally (10–15%) within Eastern and Central Pennsylvania for in-person engagements and retinal exams

What Is Required (Qualifications)

• Certified or Registered Medical Assistant or Certified EMT with 2+ years of experience
• Exceptional empathy, communication, and listening skills
• Ability to thrive in a remote work environment while collaborating with clinical teams
• Experience navigating EHR systems and Microsoft Office
• Strong attention to detail and ability to manage multiple priorities

How To Stand Out (Preferred Qualifications)

• Experience with specific EHR systems such as eClinicalWorks, Epic, Athena, or Cerner
